By completing and signing this form, I (the participant) hereby agree to the following:
I understand that there are many medical conditions that are not safe for practicing yoga or meditation. These conditions may include, but are not limited to: Asthma, Heart/Circulatory Problems, Dizzy spells/Fainting, Pregnancy, Mental Illness, High or Low Blood Pressure, Epilepsy/Seizures Neck/Back/Spine/bone problems, Slipped Spinal discs, Joint and Muscular problems, Recent surgery etc. I certify that I have no medical condition which would prevent my participation in any of the Yoga Class(es). I confirm that I am in good health and I affirm that I am personally responsible for determining if I should participate in yoga/meditation/other videos classes or not, at the time of signing this and for every time thereafter. I agree that I will consult with my physician prior to and thereafter regarding my participation in Yoga/other Class videos if I have any conditions to obtain medical clearance to practice. I AGREE NOT TO PARTICIPATE IF I AM PREGNANT OR BECOME PREGNANT AND I WILL GET FULL DOCTOR’S APPROVAL PRIOR TO JOINING POST-NATAL.
I also agree not to share these yoga classes or my online password with anyone, since participation waivers MUST be signed by ALL participants.
I confirm that I am participating in ONLINE on-demand Yoga Class(es) offered by Loretta Langille of True Healing Ocean Retreat during which I will receive information/instruction about Yoga/Meditation/Breathing techniques/Coaching and Wellness etc. I recognize that yoga/other videos may require some physical and mental/emotional exertion, which may be strenuous and may cause physical/mental and/or emotional injury. I am fully aware of the risks and/or hazards involved. I understand that the web platform(s) and app(s) used to view on-demand classes, participate in forum discussions, fill out forms on Wix website and meet for any live events may not be secure and may have privacy risks and I release Loretta Langille of any privacy or cyber liability. I understand Loretta Langille may record live yoga classes and I agree to allow my photo, video, voice or likeness to be used for business purposes. (I understand I can turn off my camera and sound if I choose but Loretta cannot manage this).
I agree to assume FULL responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in any online videos, community forum and yoga class(es) offered by Loretta Langille. I knowingly, voluntarily and irrevocably waive any claim that I may have against Loretta Langille and True Healing Ocean Retreat for any injury, death or damages that I may sustain as a result of participating in any Yoga Class(es)/videos/forum etc; including loss that may be caused by the negligence of the released party. I release and discharge Loretta Langille and True Healing Ocean Retreat from any and all liability, claim, demand or action that I may relate to the loss, theft or privacy breach that may occur during online yoga classes or any other time; and I agree to indemnify, hold harmless and agree not to sue Loretta Langille and True Healing Ocean Retreat. I, my heirs or legal representatives, forever release, waive, discharge and covenant negligence or other acts.
I have read the above release and waiver of liability and fully understand its contents. I am 19 years of age (or age of majority) or older and I voluntarily agree to the terms and conditions stated above.
